Ruth Calloway is the only nurse left in Amity Falls, holding the town's health together with duct tape and unpaid overtime since the hospital shut its doors for good. So when Halcyon Biogenix rolls into town with an "experimental blood donation drive" that pays more than most of her neighbors see in a year, she signs the consent forms herself - because someone competent has to be in the room, and because her brother Denny is first in line, chasing a cure his medical debt could never buy any other way. Ruth tells herself she's only there to watch. But the longer the drive runs, the more she notices who gets better overnight - and who stops answering the door.
Then the disappearances start. Quiet ones. Each one explained away with a shrug and a form letter about relocating for follow-up care. Ruth keeps telling herself there's a medical explanation - right up until the night she goes looking for one herself, and finds something waiting in the dark of the old mill that no consent form on earth could have prepared her for.
